Dilatation and curettage; popularly known as D and C is a procedure used for many reasons or conditions. It is known as dilatation and curettage because the cervix or the mouth of the uterus which is very tightly closed; is dilated gradually and the inner uterine cavity is cleaned  ( curetted ) to remove the tissues from inside the uterus. A dilatation and curettage ( D and C ) is primarily a diagnostic procedure to sample the inner lining of the uterus (endometrium ) which is helpful in the diagnosis of heavy or abnormal uterine bleeding. , but may also be used to treat the conditions like an incomplete miscarriage.

What is D and C

A dilatation and curettage ( D and C ) is an operation performed on women and is done under short sedation. The cervix is dilated gradually using an instrument called dilator; the endometrium or the inner lining of the uterus or the contents of the uterus are sent to the laboratory for analysis if required.

Reasons for D and C

  1. Any abnormal uterine bleeding.
  2. While investigating the causes of infertility in a woman.
  3. To investigate the cause of uterine infection.
  4. Incomplete abortion
  5. Surgical abortion

Procedure for D and C

D and C is a day care procedure in which short sedation is given the effect of which wears off in a short while. The procedure involves the following steps:

  1. Once you are anesthetised; all antiseptic precautions are taken; you will be given a special lithotomy position; and   proper draping will be done.
  2.  An instrument called speculum is inserted; the cervix is grasped.
  3. An instrument called dilator is used to slowly dilate the cervix.
  4. The uterine cavity is curetted to obtain the sample which is then sent for the investigation where it’s needed.  

Immediately after D and C

  1. Once the effects of anesthesia are gone; your gynaecologist will allow you sip of water and then light diet.
  2. You will be asked to walk around and go the wash room to confirm that you are not feeling dizzy.
  3. After confirming that you will be able to walk back without feeling giddy ; you will be discharged. The whole process takes 1.5 to 3 hours depending upon your body type.
  4. You will be advised to come for follow up  after 5 days. 

Complications of D and C

All operative procedures are accompanied by some risk; and D and C also involves few complications; still serious complications are rare if carried out by an expert at a proper medical centre with modern surgical set up where all the precautions are taken for the patient s safety.   These include:

  1. Anesthesia complications
  2. Allergic reaction of the drugs used during the procedure.
  3. Damage and trauma to the uterus; cervix or abdominal organs.
  4. Infection
  5. Bleeding

Aftercare Once D and C is Done

  1. Take care of yourself well after getting discharge. Be guided by you gynaecologist.
  2. Some cramping; spotting and lower abdominal discomfort is considered normal.
  3. You need to take mild antibiotics for 5 days.
  4. Avoid sexual intercourse for 2 weeks.
  5. Use sanitary pads instead of tampons.
  6. Avoid going for swimming.
  7. Inform your gynaecologist once you get the first period after D and C.
  8. Your gynaecologist will study your laboratory report and guide you about the further treatment.
